Never underestimate the power of the written word. I started writing campaign letters when I got a typewriter for my 9th birthday. It was to a tuna company asking them to use dolphin friendly nets. I've written many a complaint letter since then!
Companies will only change when we yell at them to do so. If not, then we'll vote with our wallets.
A great resource is The Nag - sign up and they'll remind you once a month to do something - they also provide templates and contact details.

2009 Campaigns
Vista Print: I bought my wedding invitations from them and realised that I did it without asking for their sustainability policy. Guess what, they don't have one. See the correspondence here
Ban Plastics: I wrote to Tesco, Sainbury's, Asda, Somerfield, Waitrose, Marks & Spencer and Iceland to ask them to take extreme action to ban, not only plastic bags but certain other products that contribute to plastic waste. See the letter to supermarkets here.
Abel & Cole: I wrote to my organic veg box company asking them for paperless billing, and to reduce the amount of packaging they used. I also told them my reasons for not ordering certain things from them anymore. See the letter here.
Crussh Juice Bars: I wrote to say thanks - I used my own glass jar in one of their stores, and I wrote to them encouraging them to promote it to their customers, as well as giving discounts to people who do it. See letter and their response here. See also, No Impact Man's idea here.
